AGED PEOPLE’S PERCEPTION ABOUT THE TRANSITIONAL CARE PROVIDED BY A MULTIPROFESSIONAL HOME-BASED ASSISTANCE TEAM

ABSTRACT Objective: to understand the aged people’s perception about care provided by a Multiprofessional Home-based Assistance Team in city of São Paulo - Brazil. Method: qualitative study using oral life stories. The data were collected between August 2020 and October 2021 means semi-structured interviews. sample consisted nine women assisted Multidisciplinary Team. treated according stories, presented form narratives categorized. Results: seven thematic categories emerged from narratives. participants’ health team was positive necessary, mainly because it favored access resources Unified Health System. Conclusion: importance home-based for continuity people is highlighted. reinforced need include finitude process planning actions services.
